Mental Health Training

Suite of resources and trainings to better assist individuals with co-occurring mental/behavioral and physical health conditions. This suite of materials has been curated specifically for home and community-based services (HCBS) direct care and administrative staff, to better understand and serve individuals with co-occurring physical and mental health conditions.


Recorded Mental Health Trainings

The Home Care Association Education and Research (HCA E&R) in partnership with the New York State Office of Mental Health and the Finger Lakes Geriatric Education Center of the University of Rochester provided special training and educational curriculum for staff of home care agencies, hospice, MLTC and community mental health provider agencies to advance the skills and knowledge of provider staff.

A Tool to Help you Understand Delirium, Dementia, and Depression

HCA E&R in partnership with the Finger Lakes Geriatric Education Center held an educational webinar to provide an overview of Dementia, Delirium, and Depression, and introduced a point-of-care tool to help you better understand the differences between the 3 D’s.

Innovative Hospital-Home Care Mental Health Collaboration Models: A Primer

This primer highlights hospital, home care and mental health collaboration models and efforts, including OMH initiatives that foster interdisciplinary collaboration, and OMH supportive housing provider programs that help people with behavioral health needs who are entering the community or aging in place, to be successful in the most integrated setting.

Integrated Model Care Plan for Behavioral and Physical Health in Home and Community Care along with recorded webinar. This is beneficial for provider staff as well as to improve and the agencies’ organizational practices and strategies for the care and support of patients who have mental and physical health interdisciplinary needs.
