The Home Care Association of New York State Education & Research (HCA E&R) has been awarded funding from the Mother Cabrini Health Foundation to identify and address health disparities in populations receiving health care in homes and community-based services (HCBS) statewide.

To further advance the practice knowledge of HCBS staff and inform agencies practices and strategies, HCA E&R, the NYS Office of Mental Health, and the Finger Lakes Geriatric Education Center of the University of Rochester Medical Center created an Integrated Model Care Plan for Behavioral and Physical Health in Home and Community Care. This resource is to be integrated into home health, hospice, MLTC, and community mental health provider agencies organizational practices, as well as utilized as an assistive tool and resource guide to better serve individuals with co-occurring mental health and/or substance use/addiction, and physical health conditions.

Join us as we provide an overview of the Addressing Health Disparities Through Home Care grant, the Integrated Model Care Plan, the Appendix of resources, assessments, and additional helpful links, and a companion tool template that can be used in practice. 

This webinar is for direct practice professionals as well as administrative staff.
