A Few Minutes Of Your Time: SHIN-NY Feedback

The New York eHealth Collaborative (NYeC) is currently working on a number of key SHIN-NY reform plans currently being implemented. NYeC would appreciate receiving direct feedback from Statewide Health Information Network for New York (SHIN-NY) users-- the clinicians, case managers, integration specialists, etc. 

They have compiled a brief survey which should take fewer than 5 minutes to complete; it is designed to tailor questions based on role type.
