CMS Releases OASIS E2 Draft

The U.S. Centers for Medicare and Medicaid Services (CMS) has issued the draft Outcome and Assessment Information Set (OASIS) E2 and the change table.  

The revised data set is effective April 1, 2026 and can be accessed in the downloads section of the OASIS Data Set website.

The changes include the following: 

  • A revised Transportation item (new A1255). 
  • Adding the item for Hearing (B0200) and the Item for Vision (B1000) to the resumption of care (ROC) assessment. 
  • Adds an item that replaces Gender (M0069) for the word Sex as a new item (A0810). 
  • Removed Patient’s COVID-19 vaccination is up to date item (O0350). 
  • Adds the Language item (A1110) to the ROC assessment. 
  • Modifies the skip pattern for M0102. Date of Physician-ordered Start of Care (Resumption of Care) → Skip to A1255, Transportation, if date entered. 
  • Modifies the skip pattern for M1000 From which of the following Inpatient Facilities was the patient discharged within the past 14 days? If NA is selected → Skip to B0200, Hearing. 

The OASIS E2 draft is still pending Office of Management and Budget (OMB) approval. Once cleared, the official form will display an OMB control number and expiration date. In the meantime, agencies may use the draft for training purposes.
