Now Available! Zone Tools and Patient Resources in Several Languages

A new course is now available on HCA E&R’s Learning Center containing several resources and zone tools in multiple languages.
HCA E&R was awarded funding from the Mother Cabrini Health Foundation for Addressing Health Disparities Through Home Care.
HCA E&R in partnership with Nascentia Health and several pilot agencies conducted a point-of-care translation service for English language-challenged and non-English speaking individuals receiving home care. The pilot utilized LanguageLine Solutions to provide real-time translation service right in the home over the phone.
Additionally, HCA E&R and Nascentia Health received feedback from several member agencies on what zone tools and patient educational materials would be helpful to have translated into several languages. Partnering with LanguageLine Solutions again, HCA E&R and Nascentia Health translated several resources and tools for providers and patients into various languages. Each tool has been made available in several of the listed languages.

With regard to sepsis, providers should go the further step of adopting HCANYS' clinical tools for home care sepsis screening and intervention for adults and pediatrics. Hospitals in NYS have been meeting monthly throughout the year with the state hospital association to zero in further on sepsis intervention. Home care agencies, managed care plans, and hospices can better position themselves as critical partners to hospitals, doctors and EMS in combatting sepsis, and working to save lives, help reduce sepsis’s $62 billion national cost to Medicare, and improve the health of their patients and of all New Yorkers. Home care agencies and managed care plans need to keep in mind that 87% if sepsis occurs in home and community (NOT HOSPITALS), an estimated +80% of sepsis deaths are preventable with early recognition and intervention, and the home and community based care population mirrors those at highest risk.
